The purpose of clinical incident management is to ensure implementation of consistent and accountable processes and systems for the management of clinical incidents with the goal to prevent harm to patients and improve patient safety.

Overview

Clinical incident management is an essential component of quality patient care. Most clinical incidents and near misses are caused by a chain of events and system failures, not by individuals.

Incident management is more than just a process of completing an incident report. On this page you will find education and resources to assist you to confidently navigate all aspects of incident management.

Clinical Incident Analysis

The 5 (five) Clinical Incident Analysis online learning modules are intended to support and up skill all QH staff who are involved with the management of patient safety incidents. These education modules have been specifically designed to support effective clinical incident management within each Hospital and Health Service (HHS).

Root Cause Analysis

The 2 (two) Root Cause Analysis online learning modules are intended to support and up skill all QH staff. These education modules have been specifically designed to support effective Root Cause Analysis within each Hospital and Health Service (HHS). This will ensure HHS's meet the requirements of the Patient Safety Health Service Directive and the National Safety and Quality Health Service Standards.

Open Disclosure for Health Professionals

Open disclosure is the open discussion with a patient about a clinical incident resulting in harm which was not reasonably expected as an outcome of the health care provided. Open disclosure is an honest and empathic communication to ensure patients and their families have a reliable, caring and effective way to receive factual information about the clinical incident associated with their healthcare.

Open Disclosure Consultant Training

The overarching aim of this course are: to review the formal open disclosure process and policies that are recommended within Queensland Health, to clearly define the role of an Open Disclosure Consultant by outlining the purpose and key responsibilities of this role, to explain how the Open Disclosure Consultant supports the formal open disclosure process during the preparation and facilitation of the formal meeting with the patient and/or their family and to provide participants with the skills to plan and facilitate the formal open disclosure meeting with the patient and/or family.
